(Image and Video Consent)

Our Flock - Minor Model Release (under 18 years)

I, the undersigned being over the age of 18, consent to my child/children identified below to have photographs, audio and video recordings of them taken during Our Flock photography services. I hereby give permission to the Our Flock Photographer to use my child’s/children’s photographs, audio and video recordings taken during the photography services - in original and edited forms - for promotional, marketing, and advertising purposes, which may include but are not limited to website content, social media posts, newsletters, flyers, brochures, promotional videos, and other digital or print communications. I acknowledge that neither I nor my child/children will receive any form of compensation or royalties for the use of my photographs, audio and/or video recordings in promotional, marketing and advertising materials. I understand that while my photographs, audio and/or video recordings may be used for promotional purposes, my personal information (such as my own and my child’s/children’s full name, contact information, and address) will not be disclosed. I understand that if I wish to withdraw permission for their images, audio or video to be used, I will do so by emailing info@ourflock.com.au and understand that if I do so, Our Flock will remove content from online sources, however images may appear in printed and electronic materials that have already been produced or disseminated. I understand, and agree that, despite those efforts, the images may still be published or disseminated.

By signing below, I certify that I am the parent or legal guardian of the minor/minors named below, and I have the full legal right and authority to grant the permissions and consent stated in this minor model release agreement.
